Casting in a series has been used commonly, as an intervention for reducing deformity in children and adults with orthopedic and neurologic problems, since as early as the 1950's in England and South Africa. Serial casting offers clients with joint deformity of soft tissue origin a relatively safe, nonsurgical method of regaining muscle and connective tissue extensibility and joint motion. The casting process uses the tissue's capacity to adapt to gentle elongation by growing new muscle, nerve, and skin cells, and by reorganizing the configuration of surrounding connective tissues.

Casting is often used as an adjunct to the administration of BOTOX-A and other neurolytic medications for clients with central nervous system (CNS) dysfunction. If the effect of the neurolytic falls short of the goal for recovery of joint motion, a casting series will usually successfully complete the intervention.

John Russell, CP, BOC Orthotist, of Collier Rehabilitation Systems, Inc., Pleasant Hill, CA, originated the FlexCast Preparatory AFO® fabrication technique. He often uses it to prepare the ankle and foot with equinus contracture for the introduction of an orthosis, such as his R-Wrap AFO©, by regaining joint mobility and limb volume. Beverly Cusick, PT, MS, modified Russell's technique to suit the properties of similar, adhesive-backed padding materials.
